We are currently seeking a part-time 1:1 Mentor for our Life Choices program in Burlington, MA. This position works directly with one of our individuals within the day program with significant support needs.
30 hours per week, Monday – Friday 9:00 am - 3:00 pm- Provide personal care and assistance to the individual consistent with both their physical needs and defined support plans.
- Ensure activities and trainings for the individual that enhance their Activities of Daily Living (ADL’s) and specialized service need areas with focus on continual improvement and/or maintenance of their abilities in coordination with all relevant support plans.
- Develop and implement community-based activities that promote personal choice, community inclusion, dignity, and respect while enhancing personal skill development. Provide transportation as requested by Director.
- Actively participate in interventions, providing positive, knowledgeable support to staff and managers, ensuring crisis plans and behavior support plans are followed.
- High school diploma required, Bachelor’s Degree preferred. At least one year of directly related experience working with adults with developmental disabilities.
- Valid Driver license, acceptable driving record and reliable transportation.
- Acceptable CORI report and suitable fingerprint results required.
