Teams at Yolo Hospice Inc.

Provides skilled hospice nursing care, symptom management, patient assessments, individualized care planning, education, documentation, and coordination with interdisciplinary teams, physicians, families, facilities, and community partners. Manages approximately 12 patients and conducts 3–5 visits daily across the assigned service area while ensuring regulatory and organizational compliance.

Lead an interdisciplinary hospice clinical team, oversee daily operations, mentor staff, ensure clinical excellence and compliance, manage staffing and resources, participate in QAPI, and provide after-hours on-call leadership support.
