Provide skilled hospice nursing as RN Case Manager: perform and maintain nursing assessments, coordinate medications and equipment, collaborate with interdisciplinary team, educate patients/families on symptom management and disease progression, document visits and Plans of Care within regulatory and agency standards, and recommend changes in level of care as needed.
