Lead the Connect For Life program by planning and hosting virtual and in-person events, conduct proactive outreach and home visits, document member interactions, recruit community ambassadors, connect members to health plan resources, meet engagement and attendance goals, and manage reporting and time-tracking.

Lead community engagement to build relationships, run outreach and education, connect members to healthcare and social services, close care gaps, conduct home visits and events, document interactions, and mentor/support peers.

The Consulting Physician will conduct virtual visits, assess medical needs, collaborate with care teams, and provide clinical guidance for members in a pilot program.
