Lead Wider Circle's Connect For Life program by planning and hosting virtual and in-person events, conducting one-on-one outreach and home visits, enrolling and engaging members, identifying community leaders, documenting interactions in digital systems, and meeting membership engagement and growth goals.

The Consulting Physician will conduct virtual visits, assess medical needs, collaborate with care teams, and provide clinical guidance for members in a pilot program.

Lead community outreach to engage Medicare/Medicaid populations through one-on-one and group interactions, events, home visits, and phone outreach. Build partnerships, provide culturally appropriate health education, help members access care and social services, document interactions, and mentor/support peers.

The Telehealth Physician will conduct initial virtual visits to assess medical needs and barriers, establish clinical plans, and collaborate with care teams for improved patient care.

Lead the Connect For Life program by planning and hosting virtual and in-person events, conducting outreach and home visits, enrolling and engaging members, documenting interactions in digital systems, identifying community leaders, and connecting members to health plan resources to improve wellness and attendance metrics.

Lead community outreach and engagement for older adults and disadvantaged populations. Build relationships, run events, conduct home visits and phone outreach, connect members to healthcare and social services, document interactions, mentor peers, and help close care gaps.

Provide initial telehealth evaluations for enrolled members, assess medical needs and barriers, create clinical plans, and hand off ongoing care to a multidisciplinary support team while advising on pilot program workflows.

Manage end-to-end claims and revenue cycle: review records, assign ICD-10/CPT/HCPCS codes, submit clean claims, follow up on denials and unpaid claims, handle patient billing inquiries, ensure HIPAA compliance, maintain accurate documentation, and support workflow testing and special projects in a fast-moving startup.

Build and run the clinical QA program: perform regular chart reviews for documentation and billing compliance, design QA schedules and scoring, report patterns to leadership, coordinate corrective actions with Operations, Billing, and Training, and support audits, adverse event reporting, and grievance handling.

Manage and support Wider Circle’s Connect for Life Medicaid program: supervise field teams, run onboarding and events, track member engagement, build community partnerships, ensure compliance with Medicaid contracts, and troubleshoot operational needs to meet engagement and growth goals.

The role involves conducting initial telehealth visits with members, assessing their medical needs, and collaborating with a care team to guide their healthcare journey.

Perform outreach via phone, community events, and in-person visits to educate health plan members, connect them to resources, schedule care, recruit and mentor ambassadors, track engagement in the case management system, and meet growth and retention goals while advocating for members' health.

Designs and runs onboarding, training, and ongoing enablement for Care Advocates. Creates curricula, e-learning, facilitator-led sessions, and SOPs; administers TalentLMS; tracks compliance, certifications, and readiness; partners with SMEs and Clinical Operations; analyzes training data to improve time-to-productivity, quality, and retention.
