Leads a community health engagement program by organizing events, conducting home visits and outreach, enrolling members, facilitating connections, documenting interactions, identifying community ambassadors, and helping members access healthcare services. The role requires relationship-building, public speaking, event planning, independent work, transportation for local travel, and occasional work outside regular business hours.

Build partnerships with community organizations across Northern New Jersey, conduct door-to-door outreach, lead engagement activities, establish referral channels, enroll eligible individuals, and maintain accurate visit records. The role requires representing the company, meeting weekly outreach targets, developing trusted relationships, and sharing field insights with internal teams.

The role involves conducting initial telehealth visits with members, assessing their medical needs, and collaborating with a care team to guide their healthcare journey.

Designs and runs onboarding, training, and ongoing enablement for Care Advocates. Creates curricula, e-learning, facilitator-led sessions, and SOPs; administers TalentLMS; tracks compliance, certifications, and readiness; partners with SMEs and Clinical Operations; analyzes training data to improve time-to-productivity, quality, and retention.

Build relationships with Philadelphia community organizations, senior centers, healthcare providers, faith groups, housing authorities, and other partners. Conduct door-to-door outreach, create referral channels, lead engagement activities, enroll eligible individuals, and identify adults who may benefit from services. Maintain consistent partner contact, meet weekly visit targets, document interactions promptly, represent the company’s brand, and report community insights to the team.

The Consulting Physician will conduct virtual visits, assess medical needs, collaborate with care teams, and provide clinical guidance for members in a pilot program.

Lead Wider Circles Connect For Life program by planning and hosting virtual and in-person events, conducting outreach and one-on-one home visits, enrolling and engaging members, documenting interactions, identifying community leaders, and liaising with health plans to support members' care. Track metrics, meet engagement goals, and travel locally to build and sustain community connections.

Lead community outreach and engagement for older adults and disadvantaged populations. Build relationships, run events, conduct home visits and phone outreach, connect members to healthcare and social services, document interactions, mentor peers, and help close care gaps.

Provide initial telehealth evaluations for enrolled members, assess medical needs and barriers, create clinical plans, and hand off ongoing care to a multidisciplinary support team while advising on pilot program workflows.
