Requires reliable transportation, flexible schedule, and the ability to lift supplies and communicate effectively with members.

Lead community outreach and engagement for older adults and disadvantaged populations. Build relationships, run events, conduct home visits and phone outreach, connect members to healthcare and social services, document interactions, mentor peers, and help close care gaps.

Provide initial telehealth evaluations for enrolled members, assess medical needs and barriers, create clinical plans, and hand off ongoing care to a multidisciplinary support team while advising on pilot program workflows.
