Provides outpatient case management for adults with serious mental illness, including care coordination, resource referrals, medication support and compliance monitoring, crisis assistance, basic living skills training, residential stability assessments, transportation, documentation, and individualized service planning. Works with treatment teams, residential providers, families, and community agencies to support client stability and transition to less intensive care.

Provide structured one-on-one behavioral coaching to youth and families to improve social functioning, teach interpersonal and problem-solving skills, develop individualized support plans, supervise skill practice, link to community resources, attend supervision and team meetings, and travel locally to youth homes, schools, and provider sites.

Provide outpatient case management and crisis support to seriously mentally ill adults, monitor medication compliance, develop and revise individualized service plans, coordinate community resources and transportation, maintain electronic client documentation, and assist clients and families through crises toward stable residential and recovery outcomes. Position is part-time overnight weekends.
