Lead a regional sales team to grow enhanced care management enrollment by developing regional sales strategy, securing referral partnerships, managing pipelines and metrics, coaching field Community Liaisons, and collaborating with marketing and clinical teams. Frequent travel within Northern California to support team and cultivate partnerships.

Provide remote and field-based care management for vulnerable members: assess needs, develop and update care plans, coordinate providers and community resources, conduct outreach and engagement, document in EMR/CRM, monitor outcomes, and promote mental health and recovery using evidence-based practices like Motivational Interviewing and Trauma-Informed Care.

Manages a team of 10–15 ECM staff, overseeing client outreach, enrollment, individualized care plans, KPIs, clinical sign-offs, interdisciplinary meetings, and complex case escalations. Provides clinical oversight, staff coaching, performance management, process improvement, and direct care management as needed. Collaborates with healthcare providers and organizational leaders to address medical, behavioral health, and social needs while ensuring progress toward client goals.
