The Case Manager provides comprehensive services to clients, collaborating with healthcare professionals to address clients' medical, social, and emotional needs, ensuring quality patient care and access to resources.

Provide comprehensive case management for clients living with or at risk for HIV: assess medical and psychosocial needs, develop care plans, coordinate services and community resources, perform eligibility assessments, provide crisis intervention, document activities, advocate for clients, and collaborate with interdisciplinary teams to improve health outcomes.

Lead development and execution of integrated marketing and communications to grow awareness, patient volume, philanthropy, and advocacy. Oversee brand stewardship, multi-channel campaigns, budget and performance management, market research and analytics, cross-functional collaboration, and team leadership to drive measurable impact for a multi-state nonprofit healthcare organization.
