Lead and manage the Client Scheduling Department to ensure timely, compliant scheduling and staffing. Oversee hiring, workforce planning, KPIs, process improvement, compliance audits, budget management, and cross-functional initiatives. Coach staff, drive operational efficiency, and serve as escalation point to maintain uninterrupted client services and regulatory compliance.

Provide non-clinical case management and community-based support: conduct intake, annual, monthly, and semi-annual in-home visits; update care plans with nursing; coordinate services and referrals; participate in multidisciplinary meetings and discharge planning; document per regulations; foster client advocacy, education, and safety to help clients remain at home.

Provide community-based social service coordination, conduct on-site client and caregiver evaluations, develop individualized care plans with nurses and case managers, supervise caregivers, maintain accurate documentation, assist with referrals and discharge planning, and perform field visits and replacement visits as needed.
