Teams at Victory Programs, Inc.

Provide weekly collaborative case management to ~22 families: housing search and placement, referrals, service plan development, HMIS/EHR documentation, transport clients to appointments, facilitate workshops and aftercare, deliver CSP-HI services, and support program operations and events.

Provide awake overnight direct care in a residential treatment program: monitor and document medication administration, conduct milieu rounds, maintain safety and facility standards, answer phones/front door, facilitate or co-facilitate client groups, enter client data in EHR, and provide referrals under Program Director supervision.

Provide direct care and supportive services in a residential treatment program for women with co-occurring substance use and mental health disorders. Duties include medication assistance and documentation, milieu coverage, facilitating client groups, maintaining a safe residential environment, EHR data entry, referrals, and collaborating with the treatment team.
