Provides community-based behavioral health case management and care navigation for clients and families. Conducts assessments, develops integrated care plans, coordinates medical and social services, supports benefits, housing, transportation, education, employment, and chronic disease management, and provides crisis intervention. Maintains clinical records, tracks outcomes, collaborates with multidisciplinary teams, transports clients, and advocates for client goals. Requires local community travel, documentation compliance, certifications, and knowledge of Medicaid and behavioral health requirements.

Provides psychiatric diagnostic evaluation, medication management, and crisis support for adults with severe mental illness in a community-based FACT program. Works with interdisciplinary team, documents care in the EHR, monitors medication effects, orders labs, obtains informed consent, conducts home and clinic visits, and provides on-call crisis coverage.

Supervises outpatient behavioral healthcare staff and provides clinical leadership for treatment teams serving children, families, and adults. Responsibilities include staff training, workload oversight, documentation quality, credentialing compliance, performance evaluations, peer review, and treatment team consultation. Provides intakes, treatment plans, crisis counseling, therapy, and care coordination while maintaining clinical records and professional licensure. Local travel, reliable transportation, an Oklahoma driver’s license, and ongoing agency training are required.
