Teams at Valley Falls Terrace

Manages patient and resident admissions, clinical evaluations, recertifications, baseline care plans, and quality assurance and performance improvement activities in a long-term care facility. Coordinates admissions with families, hospitals, and interdisciplinary teams; prepares QAPI reports; supports regulatory compliance, staff education, clinical meetings, and occasional nursing coverage and weekend manager-on-duty assignments.

Supervises and coaches nursing staff within an assigned unit, partnering with the Director of Nursing to ensure quality patient care, strong employee relationships, customer service, and team retention. The role requires an LPN license, supervisory experience, reliable transportation, and a stable work history.
