Teams at Urban Street Angels Inc.

Manage a caseload of 45-50 transitional age youth, conduct intakes and assessments, develop individualized case plans, coordinate referrals to housing, employment, behavioral health and community resources, monitor progress through program phases, maintain HMIS documentation, prevent terminations, and support housing stability using trauma-informed, harm-reduction approaches.

Provide employment-focused case management for transition-age youth (TAY), emphasizing trade school enrollment, job development, employer partnerships, individualized employment plans, coaching, retention support, and documentation. Manage an intensive caseload, deliver workshops, perform outreach and field visits, track retention milestones, meet grant reporting requirements, and collaborate with multidisciplinary teams to support youth toward housing stability and independent living.

Provide peer-based support, engagement, and mentorship to transition-age youth experiencing homelessness. Facilitate groups and life-skills workshops, assist with resource navigation, documentation and data collection, crisis response using trauma-informed care, and collaborate with multidisciplinary teams to support housing stability and wellness.
