Provides leadership and expert clinical guidance in an aged care facility. Responsibilities include delivering person-centered resident care, assessing complex needs, developing and reviewing care plans, mentoring nursing and care teams, managing infection prevention, supporting clinical governance and quality improvement, maintaining documentation, and collaborating with residents, families, and allied health professionals. The role requires Registered Nurse experience, aged care knowledge, and compliance with clinical and screening requirements.

Provides in-home personal care and daily living assistance to community-based clients, including hygiene, showering, meal preparation, cleaning, laundry, and ironing. Supports clients' independence through individualized care plans, social and recreational activities, shopping, appointments, and transportation. Requires a relevant Certificate III qualification, Australian driver's licence, insured vehicle, technology confidence, and successful probity checks.

Deliver person-centred wellness programs for aged-care residents, including group and individual activities that promote physical, emotional, social, and cognitive wellbeing. Build supportive relationships, encourage independence, collaborate with care teams and families, assist with wellness assessments and documentation, support volunteers, maintain safe activity spaces, and follow aged-care, infection-control, manual-handling, and organisational standards.
