Provides in-person medical care to patients in skilled nursing and rehabilitation facilities. Responsibilities include assessments, treatment planning, documentation in the EMR, chronic and acute condition management, care transitions, collaboration with facility teams, and supporting quality outcomes. The physician works independently while maintaining relationships with facility and clinical staff.

Provides evidence-based education and professional development for nursing, interdisciplinary, and non-licensed clinical staff in post-acute care facilities. Designs learning programs, addresses practice gaps, evaluates outcomes, supports competency, serves as a clinical resource, collaborates with medical and organizational stakeholders, and promotes practice improvement. Requires travel throughout the Greater Houston area.
