Provides mentoring and skill-building support to children and teenagers, helping them develop social, communication, emotional regulation, daily living, and relationship skills. Works with families and multidisciplinary teams in home, office, community, and youth-center settings. Responsibilities include crisis support, outreach, goal setting, documentation, Medicaid mental health billing, reporting, and community engagement. Requires collaboration, empathy, cultural competency, basic computer skills, and local travel.

Provides career assessments, workplace-readiness instruction, on-the-job coaching, and employment-retention support for individuals in community and educational employment programs. Collaborates with Employment Specialists and employers to identify skills, goals, and job accommodations. Maintains progress documentation and curriculum reports while supporting clients respectfully and professionally. The role requires extensive travel, driving, physical activity, and strong communication skills.

Develop and implement person-centered plans for individuals with intellectual and developmental disabilities. Coordinate health, financial, and community supports; maintain compliant documentation and outcome data; facilitate support team meetings; advocate for individuals; train and mentor staff; coordinate referrals and services; and provide temporary site supervision and on-call support.
