Provides peer support and mentoring to ACT Team clients experiencing substance use, mental health challenges, trauma, and homelessness. Delivers community-based services, harm-reduction assessments, crisis intervention, daily living skills training, resource connections, conflict resolution, and housing support. Documents incidents, collaborates with case management, maintains professional boundaries, and promotes recovery-oriented, trauma-informed care. The role is primarily field-based in the Baltimore community.
