Provide skilled nursing in patients' homes: perform initial and ongoing assessments (including OASIS), develop and revise care plans, coordinate interdisciplinary care, document clinical progress, supervise LPNs/aides, counsel patients and families, manage on-call duties, ensure payer compliance, and travel locally to patient residences.

Manage and oversee home health and hospice clinical operations, review and approve clinician documentation and orders, coordinate referrals, provide education and training, support branch leadership, participate in case conferences and quality improvement, and perform supervisory/on-call duties as needed.

Manage and coordinate hospice/home health clinical services, review and approve patient records, orders, and schedules; supervise and train clinicians; support branch director with staff management, intake/referrals, quality improvement, chart audits, on-call administration, and community education.
