Teams at Thresholds

Provide individualized employment support to people with severe mental illness using the IPS model: set goals, build resumes, prepare for interviews, remove barriers, network with employers, negotiate accommodations, and document progress while collaborating with clinical teams.

Provides community-based mental health and substance use treatment support through care planning, case management, resource coordination, daily living skills coaching, and client transportation. Builds client relationships, helps navigate healthcare and benefits systems, and supports recovery using harm reduction and trauma-informed practices. Requires adaptability, organization, strong communication, and collaboration with care teams.

Supports adults transitioning from institutional settings to community living through assessments, case management, housing assistance, advocacy, independent-living skill development, transportation, discharge planning, and linkage to services. Provides trauma-informed, strengths-based support and brief CBT/MI interventions for individuals with mental illness, substance use disorders, and physical health concerns. Completes clinical assessments, service plans, Medicaid documentation, transition-fund accounting, and required team collaboration while maintaining confidentiality and compliance.
