Teams at Thresholds

Provide recovery-oriented psychiatric evaluation and medication management for members in community settings, including home visits with ACT teams. Collaborate with multidisciplinary teams, document electronic physician notes same day, consult on treatment planning, complete member-related forms, and support training/research as directed. Must work onsite two days weekly in Kankakee, IL.

Provide community-based mental health and substance-use support through care planning, case management, and daily-living skills coaching. Connect clients to resources, transport them to appointments and benefits offices, and help navigate complex systems. Work is client-schedule driven and emphasizes harm reduction, trauma-informed care, and collaboration with clinical supervision and professional development opportunities.

Manage Thresholds' public impression and growth through communications, PR, and cause marketing. Create collateral, draft media pitches, write social posts, design materials, photograph events, run longer-term projects, and collaborate with fundraising while adopting digital marketing tools.
