Teams at The Wright Center for Community Health

Reviews and processes provider requests for patient orders, testing, and referrals; coordinates scheduling and follows up on outstanding documents, results, and patient needs. Communicates with patients, providers, staff, and outside facilities while linking documents to patient charts. Supports clinical teams, protects confidential health information under HIPAA, and ensures timely, accurate continuity of care.

Provides HIV/AIDS case management for individuals transitioning from incarceration to the community. Conducts assessments, develops care plans, coordinates medical and social services, supports treatment adherence, facilitates reentry planning, advocates for clients, maintains records, and collaborates with correctional facilities, healthcare providers, and community organizations.

Serve as front‑desk point of contact for patients: schedule and register visits, verify insurance, manage copays and cash accounts, maintain EMR records, scan and triage charts, answer phones, support extended hours and cross‑site coverage, and ensure HIPAA and compliance with center policies.
