Provides direct support to adults with developmental disabilities in center-based and community settings. Implements individualized life plans, teaches daily living and social skills, promotes independence and inclusion, documents progress, coordinates with families and interdisciplinary teams, manages activity resources, and safely transports participants while maintaining compliance with OPWDD and agency policies.

Supports adults with developmental disabilities in achieving individualized daily living, socialization, vocational, and community-inclusion goals. Provides direct support, facilitates activities, documents services and progress, maintains safety and compliance, collaborates with families and interdisciplinary teams, manages activity resources, and transports participants while promoting dignity, independence, and personal growth.

Provides one-on-one habilitation support to children and adolescents with special needs. Assesses skills, develops individualized goals, teaches independent-living behaviors, engages caregivers, implements treatment plans, documents sessions, reports progress, and participates in supervision and training. Uses a positive, strengths-based approach while protecting individuals’ rights and supporting participation in their communities.
