Provide trauma-informed, strengths-based case management for youth transitioning from hospital to community. Design and oversee CTTP program compliance with OMH, coordinate multidisciplinary care, assess needs, develop and implement service and behavior plans, deliver psychoeducation, conduct home/community visits, train and mentor staff, maintain regulatory documentation, and support applied behavior analysis systems.

Designs, implements, and documents individualized therapeutic recreation programs for residential children, assessing needs, setting measurable goals, tracking progress, coordinating activities, training staff, participating in treatment teams, and supporting community integration through events and adjunct therapies.

Support classroom teacher by reinforcing lesson plans, adapting materials, assisting with paperwork, supervising students, and occasionally covering class. Help foster academic and personal growth, manage classroom behavior, practice infection control, and build nurturing relationships with students.
