Teams at The Homeless Alliance

Provides field and home-based case management to individuals experiencing street homelessness. Responsibilities include housing applications and move-ins, housing stabilization planning, resource referrals, healthcare and behavioral health connections, documentation in HMIS, eviction prevention, advocacy, transportation, and coordination with outreach, landlords, community partners, and Continuum of Care teams. The role requires daily travel, flexible scheduling, and support for clients for one year after housing.
