Teams at THE HEALTH AND WELLNESS CENTERS, INC.

Provides peer navigation, outreach, assessment, linkage, and re-engagement services for people living with HIV who are newly diagnosed or disconnected from care. Conducts field outreach, individual and group interventions, crisis support, care coordination, advocacy, medication-adherence education, and referrals. Collaborates with clinic teams, documents client interactions, supports program evaluation, and helps clients overcome barriers to HIV treatment and support services.

Oversee daily operation and management of clinic programs and support services, ensure regulatory and payer compliance, supervise multidisciplinary staff, manage EHR/data systems and finances, lead quality improvement, emergency preparedness, community outreach, and collaborate with senior leadership on program development and grants.
