Teams at THE HEALTH AND WELLNESS CENTERS, INC.

Oversee daily operation and management of clinic programs and support services, ensure regulatory and payer compliance, supervise multidisciplinary staff, manage EHR/data systems and finances, lead quality improvement, emergency preparedness, community outreach, and collaborate with senior leadership on program development and grants.

Provide nursing case management for HIV/AIDS patients: complete assessments, develop individualized care plans, intake, document care, consult with physicians, provide education, process refills, track data, participate in QA and team meetings, ensure policy compliance.

Provide outreach, assessment, linkage, and re-engagement services to people living with HIV (PLWH). Conduct field outreach, individual and group interventions, crisis support, and liaise with testing sites and clinics to ensure clients re-enter and remain in HIV care. Document interactions, collaborate with clinic teams, and support culturally competent services.
