Prepares and serves meals, snacks, and special feedings in a skilled nursing facility. Sets up trays and dining areas, verifies meal accuracy, stores and rotates food, clears dishes, handles soiled linens, and sanitizes work areas. The role also supports residents and guests during meal service, attends required training, and performs assigned duties. Weekend, holiday, and event availability is required.

Coordinates interdisciplinary care planning and leads IDT meetings for senior living residents. Completes accurate and timely MDS assessments, selects appropriate assessment reference dates and diagnosis codes, supports PDPM classification and reimbursement, validates documentation, monitors quality measures, and maintains CMS and regulatory compliance. The role requires a Connecticut RN license, BLS certification, MDS software experience, knowledge of MDS transmission and regulations, and preferably long-term care and supervisory experience.

Prepares therapeutic desserts and salads, serves meals to senior living residents, sets up and breaks down dining areas, transports food and supplies, and maintains a clean, safe station. Follows hygiene standards, supervisor instructions, and training requirements while providing courteous service. Requires a high school diploma or GED, food preparation experience, clear communication, basic math, and dependable attendance. Senior-care or healthcare experience is preferred.
