Teams at The Center at Zaragoza

Coordinates the Resident Assessment Instrument, Minimum Data Set, Care Area Assessments, and care plans in compliance with state and federal requirements. Leads interdisciplinary assessment completion, schedules assessment reference dates, validates medical-record documentation, supports skilled utilization and case-mix management, and provides MDS and nursing documentation training. Performs additional LVN nursing duties as assigned.

Assists with meal preparation, portioning, tray and dining-room setup, food service, dishwashing, cleaning, inventory, and food-cart delivery in a residential care facility. Follows dietary, sanitation, infection-control, safety, and resident-rights procedures; coordinates with dietitians, nursing staff, residents, families, and supervisors. Requires physical mobility, frequent cart handling, lifting up to 25 pounds, and willingness to work weekends, extended hours, and emergency call-back shifts.
