Lead design and implementation of DEI initiatives to build inclusion and belonging across the organization. Develop strategy, programs, events, and metrics; ensure policy and regulatory compliance; analyze data and produce insights; collaborate with stakeholders to drive engagement and measurable outcomes.

Support outreach, intake, and engagement of children, youth, and families referred for care coordination. Document referrals, track outreach, assist EHR/utilization tasks, support fidelity reviews, and build relationships to link members to ongoing care coordination services.
