Supervises Community Integration Specialists and oversees person-centered services for adults with intellectual and developmental disabilities. Responsibilities include implementing plans, supporting community participation and employment, providing direct care and transportation, managing documentation and regulatory compliance, training staff, coordinating with families and employers, responding to emergencies, and promoting independence and inclusion.

Provides delegated nursing case management for individuals with developmental disabilities in employment, day, and daycare programs. Completes assessments, care plans, health reviews, intake documentation, and follow-up after health changes. Teaches and verifies staff competency in delegated nursing tasks, provides call coverage and emergency guidance, coordinates with families and care providers, supports interdisciplinary teams, maintains documentation, and ensures compliance with Maryland nursing and DDA requirements.

Supports individuals with intellectual and developmental disabilities in obtaining and retaining community-based employment. Provides job coaching, skills training, assessments, career planning, placement support, employer outreach, and workplace accommodations. Develops employer relationships, coordinates job opportunities, maintains EHR documentation, participates in person-centered planning, and ensures rights and supervision requirements are upheld. The role requires community travel, evening or weekend availability as needed, and collaboration with individuals, families, employers, and agency partners.
