Coordinates value-based care for primary care patients through proactive outreach, care-gap identification, utilization review, chart abstraction, referrals, and follow-up. The role monitors high-risk and complex patients, supports quality measures such as HEDIS and STARS, documents care-management activities, presents performance reports, and collaborates with clinical, administrative, managed-care, and value-based teams to improve outcomes and reduce costs.

Provides comprehensive outpatient OB/GYN care, including preventive, reproductive, obstetrical, menopausal, and gynecologic services. Performs in-office procedures such as LARC insertion and removal, colposcopies, and biopsies; interprets diagnostic testing; coordinates referrals; documents care; supports quality initiatives; and collaborates with multidisciplinary teams across outpatient locations. The role has a predictable weekday schedule with no deliveries, surgeries, inpatient duties, nights, weekends, or holidays.

Provides outpatient psychiatric evaluations, diagnoses, medication management, treatment planning, and ongoing care for patients with mental and behavioral health conditions. Collaborates with primary care and behavioral health teams in an integrated healthcare setting, coordinates referrals, documents care in electronic health records, supports quality initiatives, and may supervise trainees and behavioral health professionals.
