Lead resident care management in a skilled-care setting, coordinate with families and healthcare providers, audit charts and care plans, ensure Medicare and Medicaid compliance, and complete and transmit MDS assessments and reports.

Drive assisted living and memory care referrals and admissions by building relationships with prospects, families, and referral sources. Manage leads, tours, conversions, CRM compliance, community events, marketing, and social media to achieve census and positioning goals. Support admissions paperwork and customer service.

Prepares and serves appetizing meals according to portion-control procedures, assists with meal service and tray setup, and performs scheduled cleaning duties. Maintains timely, courteous service for nursing home residents while following food-service policies. Requires weekend and holiday availability, prolonged standing, physical ability to lift and move up to 25 pounds, attention to detail, fast-paced work, customer service skills, and a Food Handlers certificate.
