Supports daily accounting operations, including transaction entry, bank and credit card coding, accounts payable, vendor records, grant and contract invoicing, payment tracking, month-end close support, audit documentation, and discrepancy resolution. Maintains accurate records and applies GAAP, internal policies, and regulatory requirements within a preparer-reviewer accounting environment. Reports to the Accounting Manager and escalates complex issues appropriately.

Oversee the Enhanced Care Management program, ensuring DHCS and managed care compliance, billing accuracy, quality performance, and effective caseload management. Supervise and mentor a multidisciplinary care team, lead case reviews, coordinate with providers and community partners, maintain compliant documentation, monitor HEDIS and program metrics, and implement process improvements. Support person-centered care for members with complex medical, behavioral health, and social needs while maintaining service continuity and regulatory standards.

Coordinates the full lifecycle of CalAIM referrals, including intake, triage, eligibility verification, documentation, tracking, warm handoffs, and follow-up. Maintains accurate EHR and referral records, prepares reports, supports audits, and ensures HIPAA compliance and timely submissions. Serves as a liaison among members, care teams, health plans, providers, and community partners while participating in case conferences and quality improvement activities.

Provide emotional support and mentorship to wellness center participants; supervise safety and wellness checks; assist with intake, meals, medication monitoring, janitorial tasks; respond to emergencies and de-escalate crises; complete HMIS and incident documentation; facilitate life skills workshops while maintaining HIPAA confidentiality.

Provide front-line peer support at a Wellness Center, offering advocacy, crisis de-escalation, group facilitation, intake/referrals, resource navigation, documentation, and collaboration with care teams to support recovery for clients experiencing homelessness and behavioral health challenges.
