Provides direct home health and hospice patient care under registered nurse supervision. Implements care plans, delivers treatments, documents services, educates patients and caregivers, coordinates multidisciplinary care, maintains aseptic techniques, and participates in weekend, holiday, and occasional on-call rotations. Requires an Ohio LPN/LVN license, accredited practical nursing education, communication and organizational skills, and the ability to lift and transfer patients and equipment.

Provides in-home physical therapy support through therapeutic exercises, modalities, mobility training, patient reassessment, progress communication, and clinical documentation. Collaborates with physical therapists and care teams to implement rehabilitation plans and deliver personalized care. Regularly travels to patients’ homes throughout Dublin and surrounding Columbus areas. Requires an Ohio PTA license, an accredited associate degree, strong communication skills, and compassionate patient care.

Provides skilled home health nursing, including patient assessments, care planning, medication and treatment administration, direct care, education, case management, physician communication, discharge planning, and supervision of home health aides. Conducts patient visits throughout Lancaster, Ohio and surrounding areas, maintains clinical documentation, coordinates equipment and services, and participates in an occasional on-call rotation.
