Supports young adults with intellectual and developmental disabilities in building relationships, participating in community activities, developing skills, and pursuing personal goals. Coordinates community outings, mentors support staff, provides mobility and behavioral assistance, documents progress in Care Tracker, implements Individual Support Plans, and advocates for residents’ choices and independence. The role includes evening, weekend, and holiday activities, physical support, safety supervision, and collaboration with interdisciplinary teams.

Coordinates person-centered services for individuals with developmental and physical disabilities. Leads Individual Support Plan development, implementation, monitoring, documentation, and revision; collaborates with families, residential staff, clinicians, schools, and external providers; monitors outcomes and compliance; supports behavior plans; provides staff training and coaching; attends meetings, conducts observations, addresses concerns, and advocates for individuals’ needs and preferences.
