Teams at St. Croix Hospice, LLC

Provide after-hours hospice nursing: respond to triage calls, complete admissions, perform symptom management visits, adjust individualized care plans, administer medications, educate caregivers, and maintain accurate clinical documentation. Work a 7-on/7-off overnight on-call schedule and travel locally to patient homes as needed.

Lead regional hospice business development: manage BD operations and CRM, build/referral/payer relationships, develop marketing plans, set budgets and quotas, recruit and coach BD staff, negotiate service contracts, and monitor performance and market trends to achieve patient volume and revenue goals.

Provide direct patient care under RN supervision following individualized Plans of Care. Deliver treatments, manage pain and symptoms, document care, educate patients/caregivers, coordinate with interdisciplinary team, maintain HIPAA compliance, and perform home visits requiring reliable transportation.
