Teams at St. Croix Hospice, LLC

Supports a hospice bereavement program by developing care plans, conducting grief assessments and contacts, providing counseling and referrals, maintaining records, and supporting families, caregivers, and staff for 13 months after a patient’s death. The role is in-office Monday through Friday and requires HIPAA compliance, strong communication, and adherence to hospice philosophy.

Provides one-to-one hospice care to patients and families in their homes. Conducts comprehensive assessments, develops and updates individualized care plans, administers prescribed medications and treatments, educates caregivers, supports end-of-life conversations, participates in on-call rotations, and coordinates seamless care transitions with an interdisciplinary team.

Provides overnight hospice nursing care on a 7-days-on, 7-days-off schedule. Responds to triage calls, conducts urgent visits and admissions, manages symptoms, updates care plans, administers treatments, educates patients and caregivers, and documents care accurately. Requires compassionate patient support, clinical autonomy, and reliable after-hours responsiveness.
