Teams at St. Croix Hospice, LLC

Provide and coordinate bereavement services for families, caregivers and staff for 13 months after a patients death. Develop bereavement care plans, conduct assessments and contacts, offer grief counseling, maintain charts, provide referrals, and ensure HIPAA compliance.

Administer and maintain employee benefit programs (health, dental, vision, retirement, voluntary benefits). Manage enrollments, open enrollment, compliance (ERISA, ACA, COBRA, HIPAA), FMLA/workers' comp/ADA case management, vendor coordination, benefits reporting (including 5500), HRIS/payroll data maintenance, new-hire orientations, and provide employee support and policy updates.

Supervise and coordinate hospice nursing care, review referrals and records, perform/home visits, support and guide clinicians, conduct record reviews, assist with orientation, education, quality improvement, and ensure regulatory compliance.
