Provide clinical oversight and coordination of hospice patient care. Review referrals and records, conduct home visits and case conferences, assign staff, assist clinicians with care plans, perform quarterly record reviews, support orientation and education, participate in quality improvement and on-call duties, and ensure compliance with laws and organizational policies.

Provide after-hours hospice nursing care during weekend on-call shifts: triage calls, evening/weekend admissions, symptom management, medication administration, individualized care plan updates, clinical documentation, and patient/caregiver education, traveling to patient homes as needed.

The Manager of Clinical Services leads a nursing team in delivering patient-centered hospice care, mentoring, overseeing patient care coordination, and ensuring high-quality service standards.
