Provides children's mental health case management by assessing client and family needs, developing treatment and safety plans, coordinating community services, monitoring progress, maintaining records, and collaborating with providers and supervisors. The role requires compliance with Minnesota guidelines, documentation standards, clinical supervision, and continuing education. Case managers work with children and families in homes, community settings, and offices, and may transport clients to appointments.

Provide children's mental health case management: assess needs, develop and update functional assessments and Individual Treatment Plans, coordinate and monitor services, document contacts and records, develop crisis/safety plans, coordinate community referrals, consult supervisors and clinicians, transport clients as needed, and comply with Minnesota rules and clinical supervision requirements.
