Provides blended case management for behavioral health consumers, including service planning, clinical documentation, advocacy, referrals, appointment coordination, emergency response, and connection to food, shelter, medical, and social services. Coordinates with families, formal and informal supports, supervisors, and clinical staff while maintaining confidentiality and meeting direct-service productivity requirements. The role also participates in on-call coverage and may transport consumers or their belongings.

Provide psychosocial assessments, individualized service plans, supportive counseling, life skills training, advocacy for entitlements and housing, maintain client records, participate in treatment team, offer on-call support for housing emergencies, and ensure compliance with policies and regulations.

The Administrative Specialist performs various office tasks including data processing, maintaining communication with staff and clients, transcribing for medical staff, and ensuring accuracy in documentation and records while adhering to confidentiality laws.
