Teams at Southwest Care Center (SCC)

Provide HIV case management including intake, psychosocial assessment, insurance/benefits eligibility, referrals, care coordination, documentation in the EMR, prior authorizations, outreach testing, and interdisciplinary collaboration while mentoring and leading medical support staff.
