The Peer Partner provides support and resources to families involved with DCFS, conducts assessments, develops service plans, and maintains documentation, while ensuring timely outreach and follow-up.

Manage human resources functions including recruitment, onboarding, HRIS maintenance, and ensure compliance with employment laws. Responsibilities include sourcing, screening candidates, managing orientation, and contributing to policy development.

Serve as hospital point of contact to engage patients and families, coordinate services, collect and document client data, promote breastfeeding and social services, build relationships with providers, maintain resource directories, support outreach and multidisciplinary collaboration to improve patient outcomes.
