Teams at SHIELDS for Families

Serve as hospital point of contact to engage patients and families, coordinate services, collect and document client data, promote breastfeeding and social services, build relationships with providers, maintain resource directories, support outreach and multidisciplinary collaboration to improve patient outcomes.

The Navigator POSC provides prenatal and postpartum support for women with substance use disorders, conducting assessments, home visits, and connecting families to services.
