Provide home-based nursing for an adult who is trach-, G-tube- and ventilator-dependent. Work 12-hour shifts, follow ISP and physician orders, perform G-/J-tube care and medication administration, manage ventilator and trach care, maintain equipment and documentation, ensure health maintenance and safety, and assist with lifting up to 50 lbs.

Manage health, safety, and daily operations for individuals with ID/autism across residential programs. Coordinate medical care, supervise and train DSP staff, ensure regulatory compliance and accurate reporting, develop schedules, participate in on-call rotation, respond to crises, and support implementation of Individualized Support Plans.
