Teams at SFM

Provide assessments, develop and implement case and permanency plans, coordinate services and placements, support kinship/foster families, prepare youth for transition, respond to crises, participate in court and on-call rotations, supervise and transport youth, and maintain accurate case documentation.

Provide case management for children and families including assessments, placement and kinship research, case plan and permanency goal development, court reporting and testimony, crisis intervention, service coordination, supervision and transport of children, on-call rapid response with possible travel, and collaboration with teams and external agencies to achieve permanency outcomes.

Provide administrative leadership and operational oversight for the Permanency Program, supervise Permanency Supervisors, monitor program quality and fiscal performance, liaise with DFPS/courts/community partners, ensure documentation and safety, hire and evaluate staff, report critical incidents, and implement agency policies and procedures.
