Provides case management and care coordination for substance use disorder and medication-assisted treatment patients. Duties include assessments, treatment planning, referrals, motivational interviewing, patient engagement, documentation in electronic health records, coordination with medical and behavioral health teams, review of toxicology and hospital records, quality improvement, and community liaison activities. The role serves patients across multiple clinic and community locations, including hospitals and correctional facilities.

Supports nurses and clinic coordinators with non-clinical patient care and clinic management functions. Duties include processing refill requests, tracking mammograms and health maintenance indicators, routing labs, assisting with case management, completing authorization requests, updating patient records and resources, coordinating documentation, and communicating with patients, pharmacies, providers, and community services. The role requires medical assistant certification, CPR certification, strong organizational and communication skills, confidentiality, computer proficiency, and potentially bilingual Spanish-English fluency.

Support nursing and clinic coordinators with non-clinical patient care tasks including refill processing, tracking mammograms and health maintenance reports, reassigning labs, scanning/faxing, case management support, prior authorizations, maintaining community resources, and patient/provider communication.
