Teams at Santa Barbara Neighborhood Clinics (SBNC)

Provides administrative and office support to senior quality and clinical leaders. Responsibilities include calendar and meeting management, correspondence, phone and email coordination, document preparation, filing, presentations, travel arrangements, purchasing, errands, vendor communication, and confidential information handling. The role requires proficiency with Microsoft Office and Adobe Acrobat, strong communication and organizational skills, attention to detail, and a valid California driver’s license with a clean driving record.

Provides direct patient care, health education, care coordination, and chronic disease management for clinic patients. Responds to messages, coordinates follow-up after ED/hospital visits, manages medication refills per protocol, performs panel management using disease registries, and supports quality improvement and case management including occasional home visits.

Provide community outreach and education, connect patients to community services, assist with Medi-Cal and Covered California enrollment, facilitate health promotion classes and programs, represent SBNC at events, maintain program statistics, and support patient access to care.
