Provide peer-based support to residential program participants by modeling recovery skills, advocating for clients, facilitating groups, assisting with daily living and community reintegration, and supporting the treatment team in client progress and independence.

Provide case management and housing-related services to residential clients; conduct assessments and develop quarterly collaborative service plans; perform electronic charting and progress notes; maintain regular client contact, community outreach, and monitor capacity for independent living while ensuring confidentiality and compliance with hospital policies.

Provide psychosocial rehabilitation and living-skills support to residential clients, perform rehab assessments, develop recovery plans, facilitate individual and group education, complete documentation, participate in multidisciplinary teams, and provide case management and housing-related services to support client community integration.
