Teams at Saint Vincent's Services

Provide psychosocial rehabilitation and living-skills support to residential clients, perform rehab assessments, develop recovery plans, facilitate individual and group education, complete documentation, participate in multidisciplinary teams, and provide case management and housing-related services to support client community integration.

Provide psychosocial rehabilitation to clients with serious mental illness, including assessments, individualized recovery plans, group and individual living-skills training, case management, housing support, multidisciplinary team participation, and required documentation (assessments, progress notes, discharge summaries).

Provide crisis stabilization and community linkage services in an Intensive Crisis Residence: engage and assess adults in psychiatric crisis, develop aftercare plans, coordinate community resources, and perform post-discharge follow-up to support transition back to the community.
