Creates and assembles signs and sign components by handling vinyl, laminating, mounting media, trimming materials, and cutting wood, Plex, and metal. Operates latex printers, vinyl plotters, and related software while supporting shop organization. Requires production experience in a creative shop environment, familiarity with sign-making machinery and software, Microsoft Office proficiency, and strong analytical and writing skills.

Coordinates care for a large patient panel by arranging visits, laboratory monitoring, referrals, insurance verification, and community outreach. Provides patient education, conducts in-reach and external engagement, supports group visits, documents interactions in EHR and tracking systems, maintains service logs, and ensures reporting compliance. Collaborates with clinical teams, administrative staff, specialty providers, and community partners while escalating complex issues and supporting clinical program improvements.

Coordinates patient enrollment, navigation, referrals, follow-up, and retention for HIV, HCV, STI, and substance use programs. Conducts patient assessments, health education, harm reduction, street outreach, screenings, and community events. Maintains patient records, referral data, and outreach documentation while supporting HIPAA compliance and grant deliverables. Collaborates with clinical teams, emergency departments, community organizations, and recovery providers. Requires community health experience, cultural competency, flexible scheduling, and a valid California driver’s license.
