Plan and implement individualized community-based programs for infants/toddlers and families. Serve as primary family contact and conduct weekly home visits, build relationships, identify and resolve issues, maintain confidential online records, and collaborate with staff. Use Outlook, Word, Excel, and PowerPoint to document and communicate.

Provide assessment, individual and family therapy, care coordination, and tailored treatment plans for children, youth, and families across home, school, office, or community settings. Prepare clinical documentation, meet billing/productivity goals, and maintain confidential electronic records while participating in team reviews and supervision toward licensure.
