Triage and navigate clients through intake and Same Day Access, assist with EHR/patient portal setup, obtain consents and releases, coordinate care and appointments, perform outreach and re-engagement, provide post-discharge follow-up, document accurately, maintain confidentiality and trauma-informed engagement, and collaborate with interdisciplinary teams to connect clients to resources and services.
