RiseBoro Community Partnership Benefits Overview

Provides case management and supportive housing services to frail elderly tenants transitioning from shelters into permanent housing. Responsibilities include needs assessments, individualized service plans, eviction prevention, advocacy, benefits enrollment, referrals, crisis support, health coordination, home visits, documentation, tenant engagement, and collaboration with property management and community partners to promote housing stability and aging in place.

Supports school-based youth programming by coordinating activities, schedules, enrollment, events, records, reporting, compliance, budgets, staff support, participant engagement, and family communications. Assists with hiring, supervision, training, incident resolution, surveys, and maintaining program quality and performance targets.

Provides case management, needs assessments, referrals, advocacy, benefits assistance, and housing stability support for 20–25 tenant households. Maintains resident records, conducts wellness checks, coordinates home care and discharge planning, facilitates tenant activities, and collaborates with property management, community partners, and program staff. The role requires bilingual Spanish-English communication, a bachelor’s degree, and flexibility for evening and occasional weekend work.
