Provide community-based peer support, engage clients at hospital discharges and in community settings, assist with de-escalation and recovery skills, coordinate care and referrals, document services within 48 hours, participate in supervision and training, and support compliance with privacy and funder requirements.

Provide outreach, education, and enrollment assistance for Get Covered Illinois; complete state navigator training and maintain licensure; conduct canvassing, presentations, tabling, and stakeholder engagement; enroll uninsured/underinsured individuals; maintain records and reporting; work flexible hours and travel between community sites.

Provide culturally sensitive case management and outreach for Illinois Welcoming Center clients: intake and benefit assessments, case planning, referrals, advocacy, resource linkages, documentation, community engagement, and follow-up. Participate in meetings, trainings, and urgent-client support while using internal trackers and Salesforce for recordkeeping.
