Assists the Director of Nursing with supervising daily nursing operations in a long-term care facility. Responsibilities include coordinating resident care, ensuring compliance with regulations and care plans, monitoring nursing staff performance, overseeing incident reporting and patient safety, supporting committees and staff development, conducting evaluations, and providing disciplinary counseling when needed. The role also requires periodic on-call coverage and communication with physicians and families regarding significant health changes.

Coordinate and complete MDS/RAI assessments to ensure correct Medicare/Medicaid reimbursement. Audit clinical records, train staff on documentation, track benefit days and census, prepare for audits, coordinate interdisciplinary reimbursement meetings, update physician certifications and diagnoses, and support patient safety and financial management systems.

Leads the skilled nursing facility’s operational, clinical, financial, regulatory, and quality performance. Drives EBITDAR, census growth, referral relationships, admissions, resident satisfaction, survey readiness, risk management, QAPI initiatives, revenue cycle performance, and staff engagement. Oversees department managers, develops leadership talent, ensures regulatory compliance, and maintains strong resident and family experiences. Requires a valid Texas LNFA license and at least three years of skilled nursing facility administrator experience.
