Assists the Director of Nursing with daily nursing operations in a long-term care facility. Supervises nursing staff, ensures medication, treatment, documentation, safety, care-plan compliance, and regulatory adherence. Conducts rounds, coordinates committees, communicates patient changes to physicians and families, supports staff orientation and development, and participates in evaluations and disciplinary actions. Provides leadership, takes periodic call, and helps ensure consistent quality patient and resident care.

Coordinates the Resident Assessment Instrument and MDS process for long-term-care residents, ensuring timely, accurate, and compliant assessments. Audits clinical documentation, updates diagnoses and physician certifications, supports care-plan coordination, prepares for Medicaid audits, tracks benefit days and census, and collaborates with interdisciplinary and financial teams to align reimbursement with care delivery. The role also trains staff on documentation standards and maintains compliance with Medicare, Medicaid, and state and federal regulations.

Provide and oversee patient care within LVN/LPN scope: assess and document patient condition in EMR, implement care plans, administer medications/treatments, supervise CNAs, and ensure compliance with clinical policies and regulations.
