Provides recovery support and care coordination for a caseload of 160–180 members with substance use and behavioral health needs. Builds trusted relationships, uses motivational interviewing, supports treatment engagement, connects members to healthcare and community resources, navigates insurance, housing, pharmacy, and transportation systems, and coordinates referrals and documentation. The role also supports scheduling, group logistics, member communications, EHR documentation, community outreach, and interdisciplinary care-team operations.

Builds relationships, supports treatment engagement, conducts outreach, coordinates referrals, connects members with healthcare and community resources, and assists with insurance, housing, pharmacy, and transportation needs. Maintains EHR documentation, supports scheduling and treatment-related operations, facilitates community connections, and collaborates with interdisciplinary clinical teams.
