Provides community-based mentoring, care coordination, and support to youth and families. Builds positive relationships, promotes social, emotional, and behavioral well-being, collaborates with licensed professionals, and maintains professional documentation. Requires dependability, strong interpersonal skills, reliable transportation, a valid driver’s license, and automobile insurance. Prior experience in social work, counseling, human services, or a related field is preferred.

The role involves creative community-based engagement, encouragement, documentation, and helping youth recognize strengths and achieve goals.
